Some clients experience one perimenopausal skin issue&#8212;she got a full-blown skin identity crisis.</p><p>Years of detrimental skincare&#8212;scrubs, acids, retinol, perfumed products&#8212;had slowly dismantled her skin barrier. Skin, as I often say, has a long memory; it keeps score. Perimenopause simply settled it. At 58, the damage was fully visible.</p><p><strong>Skin History: Rosacea (Delayed, But Not Unexpected)</strong></p><p>Underlying redness across the forehead and cheeks appeared eight years ago, at the start of perimenopause (my client is now 58).</p><p>This is common. The groundwork is often laid years earlier, but symptoms don&#8217;t fully appear until hormonal changes tip the balance and the skin becomes thinner and more sensitive.</p><p>She showed some classic rosacea red flags:<br>&#183; Flushing after exercise<br>&#183; Fair skin that burns easily<br>&#183; A history of sun exposure in her 20s</p><p>Not an obvious textbook case&#8212;but enough. Perimenopause doesn&#8217;t create rosacea out of nowhere. It reveals it.</p><p><strong>Skin History: Acne (The Benzoyl Peroxide Rebound)</strong></p><p>Her acne story followed a familiar arc: oily, spot-prone teenage skin that eventually settled in her twenties (without medication)&#8212;until perimenopause brought it back.</p><p>This time, the spots were deeper, slower to heal, and sat beneath the surface.</p><p>Acnecide, rich in benzoyl peroxide, initially worked&#8212;as it often does. But drying out the surface of the skin comes at a cost. Strip away too much oil, and the skin loses one of its natural defences. (Our natural oil sebum) has the correct pH to help neutralise acne-causing bacteria.)</p><p>The result? Rebound breakouts and a compromised barrier.</p><p>Perimenopausal acne is not teenage acne. It&#8217;s driven by inflammation and hormonal shifts&#8212;not excess oil alone. Which means aggressive treatments tend to make it worse, not better. Treat it like you&#8217;re 16, and your skin will retaliate accordingly.</p><p><strong>Lifestyle (Not Helping, Not Hindering)</strong></p><p>An actress with both indoor and outdoor exposure&#8212;variable, but nothing extreme.</p><p>This wasn&#8217;t a lifestyle problem &#8211; this was a skin barrier problem.</p><p><strong>SKINCARE: Stop Stripping, Start Repairing</strong></p><p><strong>Cleanser</strong><br>She was using a harsh salicylic acid cleanser alongside a hot cloth cleanser packed with surfactants. In other words: too much, too harsh, too often. Stripping the skin twice, just to be sure.</p><p>This was replaced with a surfactant-free Dermaviduals DMS Gentle Cleansing Milk, which respects the skin barrier.</p><p><strong>Serum</strong><br>Her existing serum looked promising on the surface, but was largely silicone-based, with active ingredients too far down the ingredient list to make a meaningful difference.</p><p>We followed the Dermaviduals 3Rs structured barrier repair approach: Repair, Restore, Rejuvenate. Her skin was so inflamed that she required four Repair serums (10ml B; 10ml CM Glucan) before progressing to Restore.</p><p><strong>Benzoyl Peroxide (Removed Immediately)</strong><br>This was non-negotiable.</p><p>While it had initially improved her acne, it had also weakened her barrier&#8212;contributing to both redness and ongoing breakouts. Short-term gain, long-term damage.</p><p>It had effectively created the cycle she was stuck in. Very good at creating repeat business&#8212;less good for long-term skin health.</p><p><strong>Moisturiser</strong><br>She loved her existing moisturiser&#8212;and I understood why. Silicones give a smooth, semi-matte finish that sits beautifully under makeup.</p><p>But they act like cling film, trapping moisture in the short term. Over time, the skin becomes &#8220;lazy&#8221;&#8212;there&#8217;s no need to hydrate itself if silicones are doing the job. The result is dehydration and dependency.</p><p>I see it constantly. I call it <em>plastic skin</em>.</p><p>We replaced it with Dermaviduals High Classic Cream, formulated with skin-identical ingredients to support the barrier, not impersonate it.</p><p><strong>Dermaviduals Enzyme</strong><br>Although my client rarely exfoliated, she had previously overused acids. We introduced enzymes slowly to remove old skin cells (they are not dead, so we never scrub).</p><p>Her skin initially flushed, so we proceeded cautiously&#8212;no forcing, no overcorrection.</p><p><strong>5 Monthly LED Treatments (Slow and Steady)</strong></p><p>We used red, blue, and infrared wavelengths to:<br>&#183; Reduce inflammation<br>&#183; Target acne bacteria<br>&#183; Support healthy cell turnover</p><p>Her skin wasn&#8217;t immediately grateful&#8212;there was tightness and mild itching&#8212;but this settled.</p><p>By the third session, redness on the outer cheeks had begun to reduce, and breakouts became more manageable.</p><p><strong>Results (Progress, Not Perfection)</strong></p><p>At the start, her skin was tight, reactive, and uncomfortable, with painful breakouts.</p><p>By the end:<br>&#183; Skin was smoother<br>&#183; Hydration improved significantly<br>&#183; Breakouts reduced in severity and duration</p><p>There is still some residual redness, particularly around the nose and inner cheeks, which would benefit from a twice-weekly course of LED therapy.</p><p>But the real shift is structural: her skin is stronger. (Denial Is Not a Skincare Strategy)</strong></p><p>C relied on steroid cream to manage seborrheic dermatitis flares and medicated shampoo for scalp psoriasis. He hadn&#8217;t clocked that the persistent redness down the centre of the face &#8211; particularly across the nose &#8211; was rosacea.</p><p>His skin barrier was so weak that it had virtually given up.</p><p><strong>Skin History: Rosacea (A Case Study in Persistence)</strong></p><p>C ticks pretty much every rosacea box.</p><p>Like most rosacea clients, he likes to be warm. Not <em>comfortably</em> warm -aggressivelywarm. Our home is like a sauna.</p><p>He is also a committed sun worshipper. His pale skin (Fitzpatrick Type 1) burns before tanning, and yet &#8211; boldly - he doesn&#8217;t believe in SPF. He is a regular at the local tanning shop before holidays and even has a loyalty card, because apparently, natural sun damage isn&#8217;t efficient enough.</p><p>Professionally speaking, he is a challenge. Personally speaking, he is my worst client ever.</p><p><strong>Rosacea red flags included:</strong></p><p>&#183; Heat rash as a child (dead giveaway)</p><p>&#183; Fair features: blonde hair in childhood and greem eyes.</p><p>&#183; Blushed as a child</p><p>&#183; Flares: alcohol, exercise</p><p>&#183; Burns before tanning</p><p>&#183; Persistent central facial redness, especially on the nose.</p><p>&#183; A long, enthusiastic history of sun exposure and sunburn.</p><p>&#183; Regular sunbed use.</p><p><strong>Skin History: Dermatitis, Acne &amp; Scalp Psoriasis (Because One Condition Wasn&#8217;t Enough).</strong></p><p>C started early with eczema as a baby, then graduated to oily, acne-prone skin as a teenager. Impressively, this continued well into his 40s, complete with the occasional breakout and some lingering scarring.</p><p>His scalp psoriasis started as a small lump at the back of the neck, which he did what any sensible person would do and repeatedly picked. Unsurprisingly, this did not improve matters.</p><p>Flaking spread around the hairline and behind the ears. Medicated shampoo helped, but the original lump remained stubbornly unmoved.</p><p>He also had flaking across the forehead and between the brows (seborrheic dermatitis), giving the skin an overall look best described as dull, flat, and persistently irritated.</p><p>Like many clients with flaky skin conditions, he shows signs of high yeast levels and has a history of dandruff and athlete&#8217;s foot.</p><p><strong>Lifestyle (The Plot Thickens)</strong></p><p>C&#8217;s misspent youth battered his skin barrier, compounded by a nonexistent skincare routine. To his credit, he did manage to give up smoking after 25 years, which is no small feat. But his sugar addiction, combined with type 1 diabetes continue to inflame his skin internally. Skin, unfortunately, has a long memory.</p><p><strong>Treatment Plan: Repairing What Was Left of the Skin Barrier</strong></p><p><strong>Cleanser</strong></p><p>I started by removing surfactant-rich Simple micellar water, which I can only describe as diluted washing-up liquid with better branding. This was replaced with a Dermaviduals Cleansing Milk&#8212;significantly gentler and far less likely to strip what little barrier function remained.</p><p><strong>Serum</strong></p><p>Dermaviduals recommend three stages of barrier repair, but we didn&#8217;t get past the first one:</p><p>20ml Repair Serum (neat): 10ml B + 10ml CM Glucan</p><p><strong>Moisturiser</strong></p><p>Finding the right moisturiser was challenging. Like many men, he disliked anything that left his skin feeling too dewy.<strong> </strong>Dermaviduals Vitamin Lotion irritated his eyes, so we opted for their Classic Cream with a matte finish.</p><p><strong>Enzyme</strong></p><p>His skin pinked up the first few times after enzyming away the older skin cells (they&#8217;re not dead, so no acids!), but I&#8217;m sure some of those cells had been hanging on for 20 years.</p><p><strong>LED Therapy</strong></p><p>Given the state of his skin, we took a conservative route with Dermalux LED light therapy.</p><p><strong>Device:</strong> Compact Lite</p><p><strong>Light: </strong>Blue, Red, NIR</p><p><strong>Sessions:</strong> 12 treatments</p><p><strong>Duration:</strong> 4 weeks</p><p>This allowed us to:</p><blockquote><p>&#183; Reduce inflammation</p><p>&#183; Target acne-causing bacteria</p><p>&#183; Speed up his sluggish cell turnover</p></blockquote><p>Crucially, it did all of this without the collateral damage of aggressive topicals or long-term antibiotics.</p><p>The aim wasn&#8217;t just improvement&#8212;it was resilience. Skin that didn&#8217;t immediately flare at the first sign of heat, stress, or a new product.</p><p>(This didn&#8217;t stop him complaining like a child on a long car journey. &#8220;Is it done yet?&#8221;).</p><p><strong>Results (Consistency Would Have Helped.)</strong></p><p>Progress was slow initially&#8212;largely because he kept forgetting to apply his skincare. (He was so lazy that I had to massage in his moisturiser under protest).</p><p>That said, the results speak for themselves. His skin is now calm, stronger, and noticeably less red through the centre of the face. (He hates me showing these photos because he&#8217;s wearing an old t-shirt and hasn&#8217;t brushed his hair &#8211; I told him to get over it).</p><p>There is still some redness on the nose due to broken capillaries&#8212;those would require laser treatment.</p><p>But overall? A significant transformation.</p><p>He recently attended a funeral where he saw people he hadn&#8217;t seen in fifteen years. They all said he hadn&#8217;t aged a day&#8212;which I am taking full credit for.</p><p><strong>Review</strong></p><p><em>&#8220;My skin is stronger. She wasn&#8217;t a regular customer, but I always looked forward to seeing her&#8212;she lifted my spirits and made me smile.</p><p><strong>Initial Consultation</strong></p><p>The first time J came to see me, she was so nervous about changing her skincare that her skin flared up before I had even touched it. I knew we would need to take things slowly.</p><p>At the time, she was experiencing the challenges of perimenopause, dealing with both jawline acne and rosacea-like flares. Her biggest wish was to have a more even skin tone, fewer red patches, and freedom from breakouts.</p><p><strong>Skin History</strong></p><p>J has battled acne and underlying redness for decades. As a teenager and throughout her twenties and thirties, acne damaged her skin barrier. At 35, she finally found some relief with the controversial drug Roaccutane. While it cleared her acne, it also reduced her oil production&#8212;something every skin needs after 35. This may have contributed to her flare-ups during perimenopause.</p><p>(My own skin was successfully treated with Roaccutane, but it really is a last resort. Quite often, clients come to see me when Roaccutane hasn&#8217;t worked.)</p><p>When J began experiencing perimenopausal symptoms in her mid-forties, the redness she had inherited from her mother (and shared with her sister) became more noticeable. It flared after showers, exercise, or during stressful times. Around this period, her daughter underwent a major operation, and J&#8217;s stress levels were understandably sky-high.</p><p>She told me she had tried everything in the past&#8212;harsh scrubs, strong acids, retinols&#8212;but nothing worked. Instead, her barrier became weaker and more reactive.</p><p>She had a patch of eczema on her leg as a baby but no dryness now&#8212;not even tightness after cleansing. Her skin shows neither dry patches nor oily patches.</p><p><strong>Lifestyle</strong></p><p>J grew up in the Czech Republic, where her skin barrier endured freezing winters and hot summers. Fortunately, she isn&#8217;t a sun-worshipper, so sun damage wasn&#8217;t the main cause of her flare-ups. However, she noticed her skin no longer tans easily (despite being Fitzpatrick type III) and sometimes flushes before tanning&#8212;classic rosacea traits.</p><p>She&#8217;s also a keen runner, which means years of exposure to wind and weather. On top of that, her job as a teaching assistant is demanding, and during her treatment she was dealing with her daughter&#8217;s recovery as well as the threat of redundancy. It was no wonder her skin was struggling.</p><p><strong>Treatment Plan: Repairing the Skin Barrier</strong></p><p>I knew J needed a gentle approach to rebuild her barrier, rather than stripping it further. We decided on a course of LED light therapy. It kills acne-causing bacteria, reduces inflammation, and boosts cell turnover&#8212;but without the harsh side effects of strong topicals (designed for oily teenage skin) or antibiotics that disrupt the gut. More importantly, it strengthens the skin so it is less likely to flare from stress, skincare products, or heat.</p><p>J also had one stubborn, deep-rooted pore on her upper cheek that had lingered for months. To tackle it, we used a high-frequency wand before each LED session. At first, I wasn&#8217;t sure if it would budge, but by the end it was flat (though still slightly red).</p><p><strong>Results</strong></p><p>Progress was slow at first. J was understandably cautious after so many flare-ups in the past. But as she began to see changes, she started to trust the process&#8212;and me.</p><p>Her skin gradually became calmer and smoother. The difference was striking: at the start, her face was red while her neck stayed clear (a sure sign of barrier damage). By the end of treatment, her face and neck were the same tone. Her complexion looked so radiant that I even asked if she had been away on holiday&#8212;she laughed and said no!</p><p><strong>Woods Lamp at the start:</strong> dull, dehydrated.<br><strong>Woods Lamp at the end:</strong> glowing pale blue all over&#8212;perfect hydration.</p><p><strong>J&#8217;s review</strong></p><p><em>&#8220;I am not a person who seeks cosmetic care on regular basis, so to commit myself to a regular treatment and a new way of looking after my skin was quite hard for me to begin with.</em></p><p><em>Charlotte&#8217;s gentle, slow and careful approach helped me to understand my skin better and appreciate the whole healing process. The logic was simple: if you can&#8217;t frown, you can&#8217;t form frown lines, right?</p><p>Dermatologists didn&#8217;t publicly endorse it at first - it was new territory, and they were learning on the job. In reality, a skilled doctor could stop an expressive face ageing prematurely, especially if caught in the nick of time, late twenties or early thirties. Fine lines didn&#8217;t stand a chance.</p><p>Millennials, of course, dropped the discretion. They proudly rebranded Botox as a <em>&#8220;prejuvenation&#8221;</em> treatment (a blend of prevention and rejuvenation rather than correction), framing it as an almost wholesome part of their self-care routine.</p><p>And guess what? Botox isn&#8217;t just for the forehead anymore &#8212; it&#8217;s now quietly reshaping smiles, chins and even the corners of the mouth. I&#8217;ve been secretly &#8220;prejuvenating&#8221; for years, ever since a dermatologist warned me in my late twenties that I had an overly expressive face. Horrified, I tried these subtle tweaks so you don&#8217;t have to.</p><p><strong>Beyond the Forehead: The New Botox Hotspots</strong></p><p><strong>Botox for lip (perioral) lines:</strong></p><p>Pouters, beware: deep lip lines can make you look like a lifelong smoker &#8211; and they&#8217;re notoriously hard to erase. Botox can relax the muscle around the mouth that causes those vertical creases, but Dr John Burns, President of the Dallas Plastic Surgery Institute, cautions against using too much:</p><p><em>&#8220;Otherwise, you can&#8217;t pucker, and it makes you talk awkwardly.&#8221;</em></p><p>Used sparingly, Botox softens fine lines beautifully. For deeper ones, Dr Burns recommends pairing it with laser resurfacing.</p><p>I&#8217;ll admit, lip lines weren&#8217;t even on my radar until I noticed them on a peer. Botox does feel slightly restrictive at first, but yes &#8212; you can still pout (just about).</p><p><strong>The Botox Lip Flip</strong></p><p>Where did my Cupid&#8217;s bow go? By our late thirties, bone loss above the teeth can make lips curl inwards, flattening the Cupid&#8217;s bow.</p><p>A small amount of Botox injected into the central upper lip relaxes the muscle, allowing the lip to gently unfurl upward. It&#8217;s one of the rare optical illusions in aesthetics &#8212; lips appear fuller without any filler.</p><p>The results may be too subtle for trout-pouting millennials who have overdosed on filler, but for those seeking balance and softness, it&#8217;s a quiet game-changer. I tried it before it even had a name &#8211; and I still swear by it.</p><p><strong>Botox for Chin dimpling</strong></p><p>If your chin puckers when you close your mouth, Botox can help.</p><p><em>&#8220;When patients close their mouth, the skin can get dimply, almost like cellulite,&#8221; </em>says Dr Burns. <em>&#8220;Botox relaxes the muscle and smooths that look. We do a lot of that.&#8221;</em></p><p>It&#8217;s a small tweak that gives a more polished, rested look &#8212; especially when combined with other lower-face treatments.</p><p><strong>Mouth Corner Lift.</strong></p><p><em>Do you have &#8220;resting bitch face&#8221;? Over time, the corners of the mouth can droop, making you look sad or grumpy. A touch of Botox can subtly lift them.</em></p><p><em>&#8220;If you use a little bit of Botox, you don&#8217;t look quite as angry or old,&#8221; explains Dr Burns.</em></p><p>I had it done after my dermatologist noticed my lips naturally turned down. The lift is delicate but noticeable &#8212; though, as she reminded me, gravity eventually wins. Filler becomes the next step once volume loss deepens.</p><p><strong>Should Everyone Be &#8216;Prejuvenating&#8217;?</strong></p><p>Not all experts agree. Dr Uliana Gout, President of the British College of Aesthetic Medicine, notes that many lip lines stem from sun damage rather than muscle activity:</p><p><em>&#8220;So, because the first thirty years of our life we accrue all the sun damage, the UVA, the UVB, and then the skin thickens up, and you get fine line formation. The epidermis thickens, it gets more leathery-like.&#8221;</em></p><p>She supports prevention, but emphasises skin health before injectables:</p><p><em>&#8220;So, for me it&#8217;s all about diagnosing what&#8217;s happening from an external ageing perspective and internal ageing; so external factors like pollution, sun damage and diet and then internal factors like your genetic predisposition. And then when you know what&#8217;s happening there, you look at what&#8217;s happening with the skin: the dermis, epidermis. What&#8217;s happened to the muscle structure? The fat pads. Then once you narrow it down, then you know how to approach it.&#8221;</em></p><p>For her, it starts with sunscreen and exfoliation.</p><p><em>&#8220;Half the challenge we have is really bringing the skin back to normal. You wouldn&#8217;t automatically jump to toxin. You would start with resurfacing. You may not have static lines now, but you are developing early onset sign.&#8221;</em></p><p>Let&#8217;s hope millennials are using their sunblock.</p><p><strong>Can &#8216;Prejunvenation&#8217; Go Too Far?</strong></p><p>Dr Christopher Ardant, a biochemist with over twenty years in aesthetics, warns that overusing Botox can backfire. Reduced facial movement, he says, may paradoxically make a face look older.</p><p>He favours skin movement over muscle paralysis:</p><p><em>&#8220;When a subject follows a high-tech topical program containing cleansing, toning (to lower skin pH) growth factors and retinoids (especially Retinaldehyde), plus a good daily sunscreen with 18% of zinc oxide (and up), paired with chemical peels (as Modified Jessners) &#8212; they tend to abandon the use of botox, as the renewed bounciness and elasticity of the skin allows them to look attractive and younger looking.&#8221;</em></p><p>His message: address ageing at the cellular level and tweak only if necessary. Perhaps this makes him the ultimate preventionist.</p><p><strong>The Future of Prejuvenation</strong></p><p>So, is Botox the future of ageing gracefully &#8212; or just a well-branded stopgap? Perhaps it&#8217;s both.</p><p>Botox, when done conservatively, can buy time. for the past five years. Her skin is in excellent condition, but she had become concerned with the appearance of dark circles and under-eye bags. She was ready for the next stage but was sceptical about microcurrent. To ease her doubts, I treated just one eye&#8212;and we both loved the results!</p><p><strong>Microcurrent vs. Radiofrequency</strong></p><p>When it comes to lifting and firming, many clients ask whether radiofrequency (RF) is the more modern option. For Mary, I recommended microcurrent, often dubbed the &#8220;non-surgical facelift.&#8221;</p><p>Here&#8217;s why:</p><ul><li><p>Microcurrent doesn&#8217;t just tighten&#8212;it stimulates ATP, the skin cell&#8217;s energy source, for up to 72 hours post-treatment. This time, I turn to green chemist Karen Sinclair Drake and Dr. Mervyn Patterson to explore repairing your barrier the scientific way.</p><p><strong>The Truth About Ceramides</strong></p><p>Ceramides have been trending in beauty for years. This naturally occurring component of our skin has been spotlighted in the new wave of barrier creams as consumers become more aware of skin health. But here&#8217;s the catch: ceramides alone aren&#8217;t the miracle workers they&#8217;re often made out to be. They need cholesterol and fatty acids alongside them to truly restore the skin barrier. Think of them as a power trio &#8212; not solo stars.</p><p>Millward points out that skincare formulation is far more nuanced than tossing in a ceramide or two, especially when products still contain barrier disruptors like fragrance or alcohol high up on the ingredients list. <em>&#8220;Most skin returns to normal if you stop overprocessing, overcleansing, and overloading it with actives.&#8221;</em></p><p><strong>The Green Chemist&#8217;s Take</strong></p><p>Karen Sinclair Drake, award-winning green chemist and cofounder of DermOne Health and Wellness, agrees that our obsession with harsh treatments and actives is leaving skin compromised: <em>&#8220;the beauty industry has created an epidemic of sick skin because they are not focusing on the cause, just focusing on the symptoms.&#8221;</em></p><p>Healthy skin, she argues, is perfectly capable of hydrating and exfoliating itself: <em>&#8220;The skin is not a complicated organ, it just needs the perfect environment to function in a healthy way and when you give it that basic environment which is not spending 100s and 100s on all different products magic truly happens.&#8221;</em></p><p><strong>Why pH Balance Is Everything</strong></p><p>So, how do we create that perfect environment? Sinclair Drake explains that the skin barrier is strongest when it&#8217;s slightly acidic &#8212; around pH 4.7 to 5. Beneficial bacteria thrive in this acidic state, while harmful bacteria flourish in alkaline conditions. Barrier creams that overlook this balance can backfire: <em>&#8220;opening the door to unwanted bacteria. The moment that happens, it doesn&#8217;t matter what ingredients are present in the topical application, the bad bacteria will take over and the skin&#8217;s integrity will slowly break down.&#8221;</em></p><p><strong>pH Cheat Sheet</strong></p><ul><li><p>pH 4.7&#8211;5 = healthy, slightly acidic, good bacteria thrive</p></li><li><p>Neutral to alkaline = barrier weakens, bad bacteria take over</p></li></ul><p><strong>Quick Fix or Long-Term Repair?</strong></p><p>Silicone-based barrier creams don&#8217;t get Sinclair Drake&#8217;s stamp of approval. To her, they&#8217;re little more than a quick fix: unable to penetrate skin, they simply wash away at the end of the day. Instead, she advocates for organic formulas with cold-pressed oils that can absorb into the upper layers of skin and reinforce barrier function over time: <em>&#8220;the closer to nature your skin can be, the more it will stay balanced.&#8221;</em></p><p>She points out an irony worth noting: when the barrier is slightly acidic, skin can actually hydrate itself. In other words, we may not need thick, occlusive creams after all: <em>&#8220;it actually helps enhance our skin&#8217;s own natural moisturizing factor. And again that&#8217;s key when looking for a barrier cream so that means you don&#8217;t have to use the heavy types of cold cream style formula which quite frankly just smother the skin.&#8221;</em></p><p><strong>Quick Fix vs. Long Game</strong></p><ul><li><p><strong>Silicone-based creams</strong>: Occlusive, temporary, rinse away</p></li><li><p><strong>Cold-pressed oils</strong>: Absorb into skin, strengthen barrier over time</p></li></ul><p><strong>The Doctor&#8217;s Prescription for Barrier Health</strong></p><p>Dr. Mervyn Patterson, medical director of Woodford Medical, has also observed the influx of barrier creams but remains unconvinced by the hype. For him, barrier repair comes down to the correct ratio of key lipids.</p><p>In youthful skin, the barrier is built from equal parts cholesterol, ceramides, and fatty acids. (These lipids bind skin cells together, locking in moisture while keeping irritants out.) However, research from Dr. Carl Thornfeldt, founder of Epionce skincare, revealed that <em>damaged</em> skin actually needs a cholesterol-dominant formula to repair itself. Did other brands get that memo?</p><p>Delivery also matters. Without an effective system to push these lipids deeper, formulas can&#8217;t reach the basal layer where new skin cells form. As Patterson explains: <em>&#8220;But it&#8217;s not enough to have the correct formulation of the 3 key lipids, there must be effective delivery mechanisms to make sure the precursor lipids are delivered deep into the base of the basal layer so that the young epidermal cells assimilate these lipids and convert them into the correct ratio as they ascend up to become the surface roof tiles.&#8221;</em></p><p><strong>Barrier Basics: The Lipid Trio</strong></p><ul><li><p><strong>Ceramides</strong>: Help skin retain water and keep irritants out</p></li><li><p><strong>Cholesterol</strong>: Strengthens skin structure, especially in damaged barriers</p></li><li><p><strong>Fatty Acids</strong>: Support flexibility and smoothness</p></li></ul><p><strong>Prevention Isn&#8217;t Sexy, But It Works</strong></p><p>The truth? All moisturizers should contain barrier-repairing ingredients. Yet the beauty world still chases &#8220;anti-aging&#8221; miracles over barrier health. Repairing the barrier doesn&#8217;t come with flashy, immediate results &#8212; it&#8217;s about defence, not attack. She had experienced the lumps for over a year, and nothing came out when she squeezed them. To conceal the flushing, she relied on heavy Max Factor foundation, which was starting to age her.</p><p>She also began experiencing perimenopausal flushing in the evenings two years ago and stopped taking the pill one year ago, around the same time the lumps appeared &#8211; surely they must be related?</p><p>Lisa has used moisturiser daily since the age of 12 to soothe her dry skin. Several factors hinted at rosacea:</p><ul><li><p>Childhood blushing from the arms upward.</p></li><li><p>Flushing after stress, exercise, or temperature extremes.</p></li><li><p>Persistent neck and cheek redness lasting all day.</p></li><li><p>Family history of sensitive skin&#8212;her father is a redhead.</p></li></ul><p>She burned easily as a child, although she tanned afterwards, unlike her olive-skinned mother.</p><p>A Wood&#8217;s lamp examination revealed significant yeast activity. She is also prone to athlete&#8217;s foot, thrush, and dandruff, raising the possibility that the lumps were linked to a yeast-related fungal condition.</p><p>She developed eczema a few years ago after reacting to Clinique products, although she did not have eczema as a child, and it does not run in her family. Her occasional flares are easily controlled with topical steroids.</p><p>For years, she had used L&#8217;Or&#233;al cleansing milk and toner, along with Boots No.7 Protect &amp; Perfect moisturiser. The toner often left her skin feeling tight. She also exfoliated with a grainy scrub from time to time, further compromising her skin barrier.</p><div><hr></div><p><strong>Initial Skin Assessment</strong></p><p>Lisa relied on heavy, occlusive moisturisers since childhood, which softened her skin but failed to heal it. During cleansing, her skin flared red&#8212;a reaction she described as &#8220;normal.&#8221; I realised that my primary goal was long-term skin barrier repair, to reduce her flushing and reactivity.</p><p>Under LED light, her skin became red and tight and remained flushed for several hours. I advised her to take an antihistamine before her next LED session. She declined the use of Accumax, as she often forgets to take supplements.</p><div><hr></div><p><strong>Lifestyle &amp; Environmental Factors</strong></p><p>Lisa owns a hairdressing salon, leaving her regularly exposed to chemicals. She quit smoking after 25 years but rarely exercises. High stress from both work and home life remains a significant factor impacting her skin health.</p><div><hr></div><p><strong>Treatment Plan &amp; Product Strategy</strong></p><p>I planned to strengthen and repair her skin barrier over five monthly treatments, monitoring whether the lumps resolved naturally. Only then would I know her true skin &#8211; was it rosacea or just barrier damage? We combined LED, sonophoresis, and iontophoresis.</p><p>She made steady progress, but I recognised that a dedicated LED course would accelerate barrier repair. We therefore scheduled a 12-session course.</p><p><strong>Skincare strategy:</strong><br>After completing the &#8220;three Rs,&#8221; I introduced a Dermaviduals anti-rosacea serum and Vitamin Cream. She also incorporated weekly enzymatic treatments (when nagged). To test whether the lumps were yeast-related, we trialled 5ml of grapeseed oil applied directly to them. She also experimented with OMR as a winter overcoat, but this led to small breakouts, even with minimal use.</p><div><hr></div><div class="captioned-image-container"><figure><a class="image-link the one on her inner cheek resolved, revealing a mole beneath. The persistent underlying redness has now gone. The client remarked that her skin appeared so even-toned it looked as though she was wearing make-up.</p><p>Challenges included a patch of perioral dermatitis near her lips during a period of extreme stress. Her dry skin still requires an overcoat in winter, but they all block her pores, even though I told her to use a tiny amount.</p><p>Overall, the client has learned that while occlusive moisturisers provide temporary relief, they do not improve the skin&#8217;s condition long term. But does the glut of &#8216;barrier repair&#8217; creams live up to the marketing hype?</strong></p><p>When skin clinics closed during lockdown, what was a facialist to do? Educate us on Instagram, of course. To receive new posts and support my work, consider becoming a free or paid subscriber.</p></div><form class="subscription-widget-subscribe"><input type="email" class="email-input" name="email" placeholder="Type your email&#8230;" tabindex="-1"><input type="submit" class="button primary" value="Subscribe"><div class="fake-input-wrapper"><div class="fake-input"></div><div class="fake-button"></div></div></form></div></div><p>His sceptical posts about the influx of &#8216;skin barrier creams&#8217; on the market were incredibly pertinent for the many lockdowners who had gone stir-crazy and over-exfoliated their poor skin.</p><p><strong>When did we start taking our skin barrier for granted?</strong></p><p>Deep down, we know the answer. We are overprocessing our skin: over-cleansing, over-exfoliating and overloading active ingredients. One of the reasons, Andy explains, is our increasing access to ingredients previously only available under clinical supervision. Certainly, the popularity of &#8216;buffet&#8217; brands such as The Ordinary and The Inkey List has democratised skincare, and we can now formulate our own high-strength routine. &#8220;The line between professional and cosmeceutical and homecare brands is blurring all the time, and there isn&#8217;t such a distinctive difference between the two anymore,&#8221; Andy notes.</p><p>Perhaps this wouldn&#8217;t be a problem if we understood our skin. Andy added:</p><p>&#8220;The number of clients that come to me purely just for skin consultation and analysis, just so they can get a better understanding of their skin, and I&#8217;d say a good 80, maybe 90% of the time have not got a clue what their skin type is or needs. They have read articles online about a particular product that is great, but it&#8217;s whether that product is relevant to them.&#8221;</p><p><strong>So you&#8217;ve wrecked your skin barrier, and your skin has either broken out in spots or is red, sore and itchy &#8211; what do you do?</strong></p><p>Relax, Andy, I will just use one of the new barrier repair creams! Not so fast. Formulation is far more complex than just chucking in a ceramide or two.</p><p>Andy explained: &#8220;Barrier repair is a process, not a product...so for example, the thing that sparked that post was seeing people using quite stripping cleansers, they were using exfoliating acids, and then they were using a barrier repair moisturiser. Well, you&#8217;ve just created a problem and then tried to soothe it with a barrier repair cream. Just...adding ceramides into a product or D-Panthenol into a product that&#8217;s also got other barrier disruptors in there like emulsifiers or fragrances.&#8221;</p><p>He explained that there is a world of difference between chronic barrier disorders, such as rosacea or eczema and those who have just overprocessed their skin; if they had just left their skin alone, it would have righted itself.</p><p><strong>AHAs vs PHAs</strong></p><p>So, how do we use stronger ingredients safely? Acids are having a moment, but are they just a quick fix? Again, it all depends on the formulation: the pH level and the percentage.</p><p>&#8220;Yeah, so my use of acids has definitely gone down over the years. I&#8217;m not anti-acid at all. I do chemical peels, so it would be hypocritical if I were. Still, I think again that it&#8217;s an ingredient that has been overused over the years... if it&#8217;s just a 5% lactic in a toner, for example, and the pH isn&#8217;t too low, then that&#8217;s not going to be too drying.&#8221;</p><p>He prefers polyhydroxy acids, which are far more gentle than glycolic acid: &#8220;I&#8217;ve never been a big fan of glycolic anyway; glycolic isn&#8217;t something I would be personally using or recommending. My skin just freaks out glycolic; it just doesn&#8217;t like it.&#8221;</p><p><strong>Message from your skin barrier: Step away from the quick fixes.</strong></p><p>Instead, we need to stay away from quick fixes such as dermaplaing. Said Andy:</p><p>&#8220;Every time I see someone post a dermaplaning on Instagram, they are like, &#8216;Look at the glow. &#8216;Of course, the skin is glowing. You&#8217;ve just shaved the stratum corneum off. It won&#8217;t be glowing in a few days. It&#8217;s smoke and mirrors type treatment.&#8221;</p><p>Instead, non-invasive is the way to go. Before lockdown, Andy purchased a Byonik Laser, a non-wounding pulse-triggered laser, unlike many anti-ageing treatments that damage the skin, so it can repair itself. This seems to fit his ethos perfectly.</p><p>As a therapist, I couldn&#8217;t let Andy go without asking about his favourite ingredient:</p><p>&#8220;Vitamin A, probably, yeah and sunscreen, really boring, but yeah.&#8217;</p><p>Prevention is always better than cure. They roll their eyes at me and call me a nag, but there is no magic potion.</p><p>UVA (ageing) rays are present all year round and can damage your collagen even on a cloudy day. Think you are covered by the high SPF in your moisturiser? Think again. The SPF refers to UVB (burning) rays only. Skincare companies are reluctant to add too many UVA filters because they ruin the texture of their cream, and the texture can make or break sales.</p><blockquote><p>&#183; <strong>Guard your collagen with your life.</strong></p></blockquote><p>Protect the collagen in your skin from the sun, sugar, stress, and pollution. (Think of collagen as the scaffolding that props up your skin). However, skincare is only part of the solution - diet and exercise are just as important. That never goes down well. More tips as we get to know each other better!</p><blockquote><p>&#183; <strong>Proven skin-barrier-friendly ingredients</strong></p></blockquote><p>A message to skincare companies: I dare you to submit the &#8216;miracle&#8217; ingredient in your latest potion to rigorous scientific testing - you know you can afford it. &#8216;Clinically proven&#8217; is a marketing term that means nothing whatsoever. My clients might be seduced by such nonsense, but I am not.</p><p>I believe the future of skincare is skin-identical ingredients that strengthen the skin barrier and prevent &#8216;inflammaging&#8217; (premature ageing caused by sunlight, pollution, sugar and processed foods). Deep down, we all know that prevention is better than cure.</p><blockquote><p>&#183; <strong>Mediterranean anti-inflammatory diet</strong></p></blockquote><p>I&#8217;ve noticed that women are starting to realise the link between a Western diet and gut and skin inflammation &#8211; you can&#8217;t just slap on moisturiser and hope for the best. Inspired by Dr Perricone, I have been following an anti-inflammatory diet since the &#8216;90s and make sure my skincare is skin-identical to respect the microbiome.
